When you visit our office for your first orthodontic treatment, you will probably have a lot on your mind, and one of the last things you will want to do is to spend more time than necessary in the waiting room! At Orthodontic Health Center, we want to make sure that you have a positive experience at our office and have provided our patient forms here for you to download, fill out, and bring with you to your appointment. This will streamline your visit to our office, and allow our orthodontist, Dr. John Lawson, to see you sooner and get started on your new smile. We ask that you call if you are going to be more than 15 minutes late for your appointment, and we also ask that you call 24 hours in advance if you need to reschedule.
